RELIABILITY
RELIABILITY

This is concerned with human error and its sources, influencing factors, its identification / classification, qualification or quantification and its reduction or elimination. Techniques such Safety Critical Task Analysis (SCTA), a qualitative method, or HEART/THERP, quantitative methods, are used to gather and analyse task data. These techniques can then be used to influence design or be integrated as part of Hazard & Operability Studies (HAZOP) or as substantiating evidence in Safety Case orientated projects.

This is concerned with the identification, development and implementation of design requirements derived from the analysis of user tasks associated with the operation (i.e. changing, monitoring) of a system. These requirements will be derived from some form of task analysis; however there are other techniques such as operational sequence diagrams, functional adjacency diagrams and link analysis.

This is concerned with the identification, development and implementation of design requirements derived from the analysis of user tasks associated with the maintenance of system component and conditions, These requirements will be derived from some form of task analysis specifically tailored to identify and address potential maintenance errors and latent fault conditions.

This is concerned with work organisation, safe staffing levels, behavioural based safety, safety culture, working environment, work instructions / safe systems of work and more traditional safety in design.
